Seven years buying medical equipment for a mid-sized hospital network. The business covers four main segments: Imaging (MRI, CT, ultrasound, X-ray), Ultrasound, Patient Care Solutions (monitoring, anesthesia, diagnostic cardiology), and Pharmaceutical Diagnostics (contrast media).

For the "GE Healthcare USA" side specifically — the U.S. operation is their largest single market. Their headquarters moved to Chicago, and they list manufacturing and service facilities across Wisconsin, New York, and Utah. If you're a U.S. buyer, the practical thing to know is that depot repair centers and field service engineers are geographically dense in North America, which affects turnaround time on service contracts. That said, don't assume "USA" means shortest lead time for every product — some imaging components still ship from overseas.

Where I got burned: In 2019, I ordered a monitoring system assuming the U.S. business unit handled all fulfillment. Two of the modules came from a European distribution center. Added 11 days. Nobody told me because I didn't ask.

Q2: What should I check before buying a medical sterilizer?

Three things, and I learned each one the expensive way.

First, chamber dimensions versus your actual tray inventory. In September 2022, I ordered 12 sterilizers for a new maternal care wing. Everything looked fine on the spec sheet. When they arrived, the chamber was 1.2 inches too shallow for two-thirds of our existing instrument trays. The spec said "standard chamber." Our trays were apparently "non-standard." $41,000 in equipment, all returned, 3-week delay on the wing opening.

Second, cycle time under load — not empty. Vendor specs usually list cycle time for a partially loaded chamber. Full-load cycle time can be 40-60% longer. If your throughput planning assumes the empty numbers, you'll be scheduling surgeries around your sterilizer.

Third, water quality requirements. Several newer sterilizer models require specific feedwater conductivity and hardness levels. If your facility's water doesn't meet them, you're buying a water treatment system you didn't budget for. I've seen this add $8,000-$15,000 to a project nobody planned for.

Q3: Flexible endoscope procurement — what questions do buyers forget to ask?

The connector. Always the damn connector.

In early 2023, I approved an order for six flexible endoscopes from a distributor. Price was right, lead time was good, and I checked the specs myself. What I missed: the video connector type wasn't compatible with our existing processor towers. Six scopes, $28,400, sat in storage for four months while we sorted out adapters and eventually bought one new tower.

Beyond that, here's my standing checklist for flexible endoscopes:

  • Insertion tube diameter versus the narrowest anatomical pathway in your patient population. Pediatric patients need scopes under a certain threshold. Get the exact number from your clinical team before ordering.
  • Repair frequency data. Ask the vendor for average repairs-per-year per scope in facilities similar to yours. Some models are notorious for angulation wire failures within 18 months. If a vendor won't share this, that's data too.
  • Sterilization or disinfection method compatibility. Some flexible scopes are compatible with certain high-level disinfection systems and not others. This connects directly back to your sterilizer question above — don't treat them as separate decisions.
  • Service loaner availability. A flexible endoscope down for repair without a loaner means cancelled procedures. Ask what the loaner pool looks like in your region before you sign.

Q4: How does an artificial heart work?

Not a procurement question, but I've been asked this at least four times by colleagues, so here it is.

An artificial heart — technically a total artificial heart (TAH) — replaces both ventricles of a failing heart. The most well-known version is the SynCardia TAH. It works by using pneumatic pumps to push blood from the device into the pulmonary artery (to the lungs) and the aorta (to the body). An external driver unit, about the size of a briefcase, powers it through tubes that pass through the patient's skin.

There's also a second category called ventricular assist devices (VADs), like the HeartMate 3. These don't replace the heart — they assist one ventricle. The HeartMate 3 uses a magnetically levitated rotor spinning at 5,000–6,000 RPM to produce continuous (not pulsatile) blood flow. Patients with a VAD may not have a detectable pulse.

The point worth knowing from a hospital procurement angle: these devices require dedicated monitoring infrastructure, specialized ICU beds, and anticoagulation management protocols. If your facility is considering a VAD program, the device cost is only roughly 30-40% of the total program cost. The rest is training, monitoring equipment, and pharmacy overhead.

Q5: What's the mistake that cost the most?

A service contract I didn't read closely enough.

In 2021, we signed a 3-year service agreement on imaging equipment. Looked standard. What I didn't catch: the uptime guarantee excluded "consumable-related downtime," which the vendor defined broadly enough to cover most of our actual downtime events. I tracked it for 14 months. 62% of our downtime hours fell into the excluded category. The contract guaranteed 97% uptime in theory and delivered 91% in practice — and there was nothing to escalate because the exclusion was in the fine print.

From the outside, a service contract looks like a maintenance plan. The reality is it's a risk-allocation document, and the vendor has lawyers making sure the risk allocates to you. I now have a paralegal review every service contract and highlight every exclusion clause in yellow before I sign anything.

Q6: Any final advice for first-time equipment buyers?

Ask vendors for the things they don't put in the brochure:

  • Uptime data from facilities similar to yours (not the "best case" reference sites they offer)
  • Full-loaded cycle times, throughput limits, and utility requirements
  • Compatibility with your existing ecosystem — connectors, interfaces, software versions
  • Service contract exclusions in plain English

An informed customer asks better questions and makes faster decisions. I'd rather spend 20 minutes going through this list than three weeks explaining to my director why $28,000 worth of endoscopes are sitting in a storage closet.
