I've become an accidental expert in areas I never planned for: dialysate water flow, sterilizer load configurations, and why a 350-pound electric wheelchair requires a ramp gradient certificate before you order it.

If you're stepping into a similar role, this checklist is for you. It's the 5-step process I run on every significant equipment purchase. It came from trial and error—more error than I'd like to admit.

Step 1: Confirm who you're buying from before you compare prices

Gray-market medical equipment is a bigger problem than most administrators realize. The machine might work perfectly. Or it might arrive without proper regulatory approval for your region, no warranty, and firmware that can't be updated. I learned this the hard way when a vendor's "great deal" on patient monitors vanished into a warranty dispute.

Here's the verification checklist I now run:

  • Ask for the manufacturer's certificate of authenticity. Every major OEM can provide one. If a vendor hesitates, that's your answer.
  • Check the seller against the manufacturer's public partner list. GE Healthcare, for instance, lists their authorized distributors on their website. If the company isn't listed, find out why.
  • Compare branding carefully. This sounds trivial, but it's saved me more than once. When I put together a bid-comparison document last year, I downloaded the official GE Healthcare logo—the transparent version is available in their brand and press assets—and used it to check each vendor's paperwork. One quote had a slightly distorted logo, stretched and low-resolution. That vendor turned out to be buying through an unauthorized importer.

Per FTC advertising guidelines (ftc.gov), performance claims have to be substantiated. That's not just legal language. It means you can demand documented evidence for any spec a vendor gives you. If they can't produce it, what else are they skipping?

Step 2: Audit your building before you budget for equipment

This is the step most facilities skip. It's also the most expensive one to skip.

Before requesting a quote, know what your building can handle. If the equipment has connectivity features—and almost everything does now—verify your network infrastructure against the manufacturer's requirements. GE Healthcare's fiber network build specifications for imaging and monitoring systems, for example, include minimum bandwidth, latency thresholds, and redundancy requirements. If your network was installed when the building went up in 2006, it might not make the cut.

We found this out when a new ultrasound system kept freezing during image transfers to PACS. The vendor's equipment was fine. The building's network wasn't. We paid for a fiber network upgrade that was never in the budget because, honestly, nobody had thought to check.

If I remember correctly, the network assessment took about three weeks. The upgrade took another six. And we had a $90,000 ultrasound system sitting in its crate for two months. The rental we needed in the meantime? $4,200. A lot cheaper than the upgrade. But entirely avoidable if we'd checked before we bought.

Power, water, space—same deal. I want to say the electrical and plumbing work for a new dialysis machine cost us $14,000. Actually, $19,000 once we included permits and a contractor's change order. The machine's list price was only part of the story.

One more thing on timing: if your go-live date is fixed, budget for delivery certainty. In March 2024, we paid $900 extra for expedited contractor work on a fiber connection because the imaging system's training schedule was locked in. Rescheduling the radiologist's team would have cost far more in internal disruption and lost imaging revenue. That's the kind of math that justifies itself.

Step 3: Learn the basics of the technology—even if you're not clinical

You don't need to be a doctor to buy medical equipment. But you need to understand what the device does, what it needs to function, and what it consumes over time. Otherwise you're just accepting whatever the vendor tells you.

Take hemodialysis equipment—one of the most intimidating purchases from an admin's perspective. How does hemodialysis work? A patient's blood is pumped through a dialyzer, a filter containing a semipermeable membrane. Waste products and excess fluid cross that membrane into a dialysis solution, and the cleaned blood returns to the body. Simple in theory. But the equipment required to support that process is the real procurement story: a water purification system, a dialysate delivery system, power backup, disposables, and trained technicians.

Ask your clinical team to walk you through a typical procedure. Ask the vendor for training materials. Then ask the critical question: what breaks, what gets consumed, and what does it cost per month? These questions sound obvious and still get skipped in the excitement of a capital purchase.

Step 4: Plan for sterile barrier compliance as a line item

Infection control purchasing doesn't get attention until an auditor notices a problem. That's wrong. Sterile barrier systems—the packaging that keeps instruments sterile until the moment of use—are a real cost center with real compliance requirements.

Here's what to check on any device that touches a sterile field:

  • Sterile barrier material standards. Ask vendors which ISO standards apply. Products that can't demonstrate compliance will fail validation.
  • Testing requirements. Does your facility have the equipment to perform peel-pouch tests and seal validation? If not, that's a cost to include.
  • Disposables vs. reusables. Understanding what's single-use versus reusable changes monthly supply budgets significantly.

The most frustrating part of sterile barrier procurement is that issues don't show up immediately. It takes a batch of rejected packs, a failed validation audit, and suddenly you're explaining to your CFO why the sterilization department needs an unexpected $25,000 for new equipment and testing. I've lived that meeting.

Step 5: Don't let departments buy mobility equipment in silos

Electric wheelchairs and transport chairs sound simple compared to imaging systems. They're not. When different departments buy different models on their own, you end up with a mixed fleet of battery types, chargers, and service vendors. That's a maintenance nightmare hiding in plain sight.

When we consolidated our electric wheelchair procurement to two standardized models, the maintenance costs dropped noticeably. Same charging infrastructure. Same spare parts. One service vendor who actually showed up on time because the volume justified it.

The mistakes I keep seeing—and the fixes

Real talk: most equipment procurement problems trace back to a missing process, not a missing budget. We didn't have a formal approval chain for rush orders until an unauthorized expedite fee ended up on an invoice. Finance rejected it. I had to explain to my VP why our department absorbed a $2,100 charge that should never have happened. Now there's a one-page form for every equipment order above $1,000. It's not bureaucratic theater—it's accountability.

The other recurring issue is treating delivery promises as facts. Vendors are optimists by nature. When a device is offline and patients are scheduled, certainty matters more than shipping cost.
