I'm a quality and brand compliance manager at a medical equipment company. In my role, I review roughly 200 unique devices a year before they ship to customers. Over four years, a pattern has become predictable: buyers get excited about spec sheets, order on price, and only later discover the equipment costs far more to own than it cost to buy.

This checklist is for procurement leads, clinic owners, and hospital administrators making a purchase decision—whether that's a CPAP machine for a sleep lab, bedside monitors for a patient floor, or hearing aid programmers for an audiology practice. There are seven verification points. Work through them in order, before you commit to a quote.

Before You Even Look at Pricing

1. Confirm what the device needs to do for your workflow

Take a CPAP machine, for example. A CPAP machine works by blowing a continuous stream of pressurized air through a mask, which keeps the patient's airway from collapsing during sleep—it's the standard first-line treatment for obstructive sleep apnea. The problem is that two CPAP machines can look identical across vendors while behaving very differently in daily use. Do you need auto-adjusting pressure or fixed pressure? Does it log compliance data for remote review? Can your staff configure it without a week of vendor training? Write down your clinical and workflow requirements before you talk to a single sales rep.

Bedside monitors are no different. I've watched buyers select a base configuration and discover later that SpO2 monitoring—a core need on any general patient floor—was a paid add-on module. The "affordable" monitor suddenly costs $1,800 more per bed. That's not a pricing surprise. That's a requirements gap.

2. Verify regulatory compliance yourself

Don't take a salesperson's word that a device is "certified." Ask for the actual documentation. For medical electrical equipment, you want to see IEC 60601-1 certification (the international safety standard for medical electrical equipment). You also want to check the local registration status—in the U.S., that means FDA clearance or approval; in Mexico, it means COFEPRIS registration. A device that's perfectly approved in one country may still be pending in another.

When we ran our Q1 2024 quality audit, 12% of vendor compliance documents we collected were either expired or didn't match the device revision on offer. That's roughly one in eight. It's rarely malicious—vendors have multiple document versions floating around—but it means those devices could face delays at the border or be non-compliant for the intended clinical use. It's also worth asking whether the manufacturer operates under ISO 13485, the quality management standard for medical devices. If the answer is unclear, that's a red flag.

3. Read the warranty line by line

Here's something vendors won't tell you: the standard warranty is written to protect the manufacturer, not you. Most buyers focus on warranty length (one year vs. three years) and completely miss the exclusions. Calibration is often not covered. Power-surge damage is frequently excluded. Consumables and accessories almost never are.

On a hearing aid programmer, a damaged probe cable is a $200+ part. On a bedside monitor, a screen replacement can be a major line item. If your warranty excludes these (and most do), you need to budget for them as routine operating costs. Which, honestly, most procurement teams don't.

4. Calculate total cost of ownership, not just the unit price

The sticker price is the smallest number you'll see. The five-year cost of owning a medical device includes shipping, installation, training, service contracts, calibration, consumables, and the cost of downtime when a device fails. A $4,000 bedside monitor with a $1,200 annual service contract is a $10,000 decision over five years. Add calibration on top of that, and the number looks very different from the original quote.

I went back and forth between two vendors on a monitor replacement project for three weeks. The established vendor had a higher upfront quote but a solid service contract. The newcomer was roughly 25% cheaper on paper. Ultimately I chose the established option because the project was for a cardiac unit where downtime risk outweighed the savings. It wasn't an exciting decision, but it was the right one—and it's why I now calculate total cost of ownership before comparing any vendor quotes.

5. Test the service network—before you need it

Call the service number on the vendor's website. Time how long it takes to reach a real human. Ask about average response time in your region, and ask what the maximum response time has been in the past year—that last number tells you more than the average. I evaluate vendors for a living (including the one I work for), and service depth is the single biggest predictor of long-term buyer satisfaction I've observed.

Take GE Healthcare in Mexico as an example. The company's service network is one of the reasons its reputation has held up well into 2026. But even a strong national network has regional variation. A vendor that's excellent in Mexico City might be slow in a rural clinic, or vice versa. Don't accept a general "we have great service" claim. Ask for a response-time commitment in writing, specific to your location and the device you're buying.

6. Ask for a reference site—or a hands-on test

Spec sheets are marketing documents. I've learned to ask where the device is installed near me and to call those customers myself. The question everyone asks is "what's the price?" The question they should ask is "how does this device perform in a real clinical workflow?"

When I ran a blind test with our clinical team comparing two similar devices, 85% identified one as more professional without knowing which was which. The difference wasn't which device looked better on paper—it was which one was easier to set up and calibrate consistently. If a vendor won't provide a reference site or a demo unit, treat that as a red flag.

7. Plan for the whole lifecycle

Medical devices need calibration. Depending on the device type, that's annual or semi-annual. Don't hold me to a universal number—it genuinely varies by manufacturer and use—but every vendor should be able to give you a calibration schedule and cost in writing. Also ask about software updates. Hearing aid programmers are essentially software tools; they connect to a PC, run configuration software, and need updates when operating systems change. Bedside monitors receive firmware updates that affect security, connectivity, and performance. If those updates aren't included in the service contract, they're a future cost that's easy to overlook.

A piece of medical equipment isn't a one-time purchase. It's a six-to-eight-year relationship. The question every buyer should ask—but rarely does—is simple: "What does this device look like to own in year five?"

Mistakes I Still See Buyers Make

Even experienced procurement teams slip on the same items. Here are the most common ones from my audits.

  • Buying on spec-sheet interpretation instead of clinical validation. If your respiratory therapists can't easily configure the CPAP machine, no feature list is going to save you.
  • Forgetting installation costs. Wall-mounted bedside monitors need electrical work, mounting hardware, and often network cabling. I've seen these add 15–20% to the project cost.
  • Comparing first-year costs instead of five-year costs. A device with a lower upfront price but a higher service contract is rarely the better deal.
  • Assuming training is included. Some vendors include basic training. Advanced training is often a separate—and significant—line item. Ask explicitly.
  • Not checking consumable pricing. A CPAP machine runs through filters, masks, and tubing, and replacement costs add up quickly. Ask for a consumable price list before you commit.

The medical equipment market isn't getting simpler. Product lines keep expanding, and the line between hardware and software keeps blurring. But if you work through these seven verification points, you'll be ahead of most buyers I've encountered—and you'll know exactly what your equipment really costs over its entire life.
