I'm an office administrator for a ~200-person multi-specialty clinic. I've handled purchasing since 2020—roughly $1.1M annually across 30 suppliers for everything from surgical gowns to a patient monitoring system. I report to both operations and finance, so I get asked to make things work and keep costs defensible. I'm not a clinician or a biomedical engineer. This is the procurement checklist I actually use.

If you're an admin, operations lead, or practice manager responsible for equipment and supply buying, this is for you. It covers six steps: vendor profiling, total cost, compliance, service, pilot orders, and file management. It's not a clinical evaluation guide.

From a procurement perspective, my job is to verify that what the salesperson says matches what the contract says—and then make sure the invoice doesn't surprise finance.

Step 1: Pull up the vendor's company profile before you look at product specs

The first thing I do is check the company profile, not the product page. When I was comparing options for a patient monitoring system, I looked at the GE Healthcare company profile first. I wanted to see whether the vendor had the full ecosystem—monitoring, imaging, diagnostics, service—or whether I'd have to manage three separate vendors for one integrated deployment. GE Healthcare's profile showed a broad installed base and a service network. That matters when you're trying to get a 24/7 system repaired.

What to check in a company profile: product categories, service footprint, software platform, continued investment in R&D, and whether they publish regulatory or compliance information. If the profile is only marketing copy, that's a red flag.

Step 2: Compare total cost of ownership, not just the quote

This is where I've made my biggest mistakes. Early on, I took the lowest unit price for what I thought was an apples-to-apples comparison. It wasn't.

For a patient monitoring system, the quote is only a starting point. The total cost of ownership includes:

  • Base hardware and software licenses
  • Integration with your existing EHR or hospital systems
  • Installation and network setup
  • Staff training
  • Service contract and response-time guarantee
  • Accessories (cables, sensors, batteries) and their replacement cycle
  • Downtime risk if the vendor has weak local support

So now I build a simple TCO table before comparing vendors. I went back and forth between an established vendor and a new one for two weeks. The established one offered reliability; the new one offered 25% savings. In the end, I chose the established vendor because the project was too important to risk on an unproven service model. Even after choosing them, I kept second-guessing. What if the new vendor was actually fine? The two weeks until the system was installed were stressful. But the TCO table gave me a rational answer.

To be fair, price matters. I'm not saying ignore it. But compare price after you've defined the spec—otherwise you're just picking the cheapest surprise.

Step 3: Verify standards and claims before you buy surgical gowns

Surgical gowns are a product category where buyers can easily under-spec. I'm not a clinician, so I can't speak to every barrier requirement. What I can tell you from a procurement perspective is to ask the vendor to document the standard they're claiming.

For a surgical gown, look for AAMI PB70 barrier levels. A Level 1 gown is not the same as a Level 4 gown, and the price should reflect what your procedure actually needs. If a supplier says a gown is 'sterile', 'fluid resistant', or 'high barrier', they should be able to show the test method and the level. Per Federal Trade Commission (FTC) advertising guidelines, claims have to be truthful, not misleading, and substantiated with evidence. A vague product sheet is not evidence.

The most frustrating part of buying surgical gowns: the same issues recurring despite clear communication. You'd think written specs would prevent misunderstandings, but interpretation varies wildly by vendor.

Also check where the gowns are manufactured and whether there's a lead-time risk. That's a supply-chain conversation, not a clinical one—and it belongs in your TCO model.

Step 4: If it's new to you, start with 'what is a dental air compressor' before you buy one

This is the kind of product that feels simple until you get the installation bill. When our dental clinic asked me to source a dental air compressor, I honestly didn't know the first thing about them. I'm not a dental engineer. So I researched it, and here's the short version: a dental air compressor supplies compressed air to handpieces, scalers, air-water syringes, and sometimes lab equipment.

Why does that matter for purchasing? Because the compressor itself is only part of the system. You also need filtration and dryers to keep oil, water, and debris out of the air going into patients' mouths. Oil-lubricated compressors tend to be cheaper upfront, but they need more maintenance and filtration. Oil-free models cost more but can be simpler to maintain—and they avoid certain contamination risks.

Ask these questions before you get a quote: What's the required CFM at your chair count? Is the compressor oil-free or oil-lubricated? What's the next maintenance interval? Is installation included? Where does the unit sit, and how loud is it? What's the replacement filter cost? A low quote that omits the dryer and filters will end up costing more—another example of the total-cost trap.

If you're a large health system, you might have a biomedical engineer on staff. I don't, so I rely on vendor documentation and a service contract. Either way, this is a step where procurement should not guess.

Step 5: Run a small pilot order before committing to a high-volume product

When the clinical team asked me to standardize surgical gowns, I didn't go straight to a year-long contract. I ordered a small lot under regular shipping, handed it to the OR manager, and asked for feedback. That pilot caught a fit issue that would have gone unnoticed if I'd only looked at the spec sheet.

This also works for monitoring systems, but it's more complicated. You can't pilot a full patient monitoring system without integration. What you can do is ask for a reference site, speak to a current customer, and ask for a demonstration at your facility with your network environment.

If the vendor won't do a pilot or a reference call, that's a deal-breaker. In my experience, the vendors who say 'trust us' are the ones whose sales pitch exceeds their follow-through.

Step 6: File support contacts before you need them

The step everyone ignores is documenting after-sale contacts. With GE Healthcare equipment, for example, there's a difference between the field service line and the benefits/HR line. If a former employee is trying to reach their retirement plan, the GE Healthcare pension master phone number should be in the benefits portal or plan documents—not on an equipment label. (Should mention: this also applies to other large vendors. Put the right phone numbers in your vendor file before someone calls the wrong line and wastes an hour.)

For every major purchase, I create a one-page vendor file with:

  • Sales contact

  • Billing contact
  • Installation and training contacts
  • Warranty expiry date
  • Contract renewal date

That one page has saved me more money than any discount negotiation. It prevents urgent calls from going to the wrong place, and it makes it easier to audit costs at renewal.

Common mistakes and final reminders

  • Don't compare quotes until you've written the spec. 'Cheaper' means nothing if one vendor includes installation and training and the other doesn't.
  • Don't assume the warranty covers everything. Ask what's excluded.
  • Don't skip the support file. It's not glamorous, but it's the difference between a five-minute invoice issue and a two-week accounts-receivable problem.

My experience is based on about 30 vendors, mostly mid-range B2B suppliers. If you're at a huge health system negotiating through a GPO (group purchasing organization), this checklist will feel basic. If you're in a small clinic, it will probably feel familiar. In either case, the principle is the same: the lowest quote is not the lowest total cost.

Bottom line: buy with your eyes open. Use the company profile to judge the vendor, use TCO to judge the deal, use standards to judge the product, and use the support file to make sure the relationship doesn't fall apart after the invoice is paid.
